Undergraduate Minor in Education
Completing the Minor
Students who complete the requirements for the Education Minor will receive an official completion letter in the US mail. In addition, within a couple of months after receiving the letter, a notation will appear at the bottom of your transcript that will state "MINOR PROGRAM IN EDUCATION COMPLETED" with the semester and year. However, to initiate both the letter and the notation, Education Minor students must submit the following items to the Minor Advisor. (L&S students will have an additional form to complete - see below):
ON LINE VERIFICATION OF COMPLETION FORM
This form, which is completed on line using your CalNet ID, lists the courses and fieldwork you have completed for the Minor, with the grades received, and is your official record of Minor completion for the Office of the Registrar. You do not need to submit a transcript. When you are ready to complete this form, you should have access to your transcript to make sure you give accurate information about which courses you took, when you took them and the grades. The Minor Advisor will have access to and will check the information you have entered after submission.
If you are completing your Minor requirements in the same semester in which you are graduating, you are encouraged to submit the Verification form before the end of your last semester. You can leave blank the grade(s) for the course(s) you have not yet completed. They will be entered by the Minor Advisor.
Important items to note while completing this form:
• Course in common: If you will be using one course in common between the Education Minor and your major or other minor, please make sure you select the button in front of the course you will be using in common. If you are not using any course in common, you don’t need to select anything.
• Fieldwork: When entering your fieldwork, please make sure you enter the section number if your fieldwork was under EDUC 97 or 197. If you are not certain of the section number (they are not listed on your transcript) you can refer to the list of field studies sections on the requirements page of this website. For the title of your fieldwork, you can state the name of the program, the location, or “field work”.
• Submit button: This form does not save anything you have entered until you click the “submit” button. If you forget to click “submit”, nothing you have entered will be saved or sent to the Advisor. Please make sure your information is accurate before you click the “submit” button, as you cannot change anything you have entered once you have selected that button. If you have made a mistake or wish to change something after submitting the form, please notify the Advisor.
• Change of contact information: If the address, phone number or other information listed under your name is incorrect, please notify the Minor Advisor.

EXIT SURVEY
Education Minor students must also submit an Education Minor Exit Survey. This survey gives the Education Minor Committee valuable feedback on various aspects of the Minor, enabling an attractive program to continue to be offered to our students. Candid responses are greatly appreciated. Responses will be compiled each semester for review. Students complete the survey on line, using their CalNet ID, at this link: Education Minor Online Exit Survey. The Minor Advisor is automatically notified once you have completed the survey. Questions about the survey should be directed to the MinorAdvisor.
FOR ALL L&S STUDENTS
L&S students completing the Education Minor must verify that they have completed at least four courses for the Minor, either lower or upper division, that are not required by their major or other minor.
When L&S students submit the on line “Verification of Completion Form” to the Education Minor adviser, students must also submit a Major/Minor Course Overlap Check form (PDF) . The form is also available on the L&S Forms & Petitions website: http://ls-advise.berkeley.edu/fp/fp.html, as Page 2 of the Completion of L&S Minor packet. Hard copies are available in 1600 Tolman.
Instructions are included with the form. Please note that you will need to obtain the signatures of your L&S major adviser(s) and any other minor advisers on the Overlap Check form(s). If your major and minors are in very different fields and there is no possibility of overlapping courses, check with each minor adviser about waiving the signature of your major adviser and other minor adviser.
